    Carrier Contract Forms: LOA and Street Level Overview

    When sending out carrier contract forms, it's important to choose the correct Level of the Downline agent.

    Street Level

    Choose Street to generate a unique link.

    A link is generated to send to the Street level agent.

    Confirm your Direct upline and that your level is "Street". Next, click Get started.

    LOA Level

    Choose License only agent (LOA) to generate a unique link.

    A link is generated to send to the LOA level agent.

    The LOA will confirm their Direct upline and that they are at the “LOA” level.

    Agents will input their NPN to verify their information.

    Next, they'll add their:

    • First Name
    • Last Name
    • Last 4 of their SSN

    During the carrier request process, choose between the following statuses: New, Transfer, or Ignore.

    Confirm if you've completed this year's AHIP certification and then select Continue.

    Acknowledge the information provided is correct. Agents are also able to make any edits to the requested carriers.

    Finally, sign documents and review any outstanding next steps/action items.
